Ground breaking diabetes education booking platform

The Challenge

National audit data (2016) showed uptake of diabetes education is poor (8.3% of eligible adults with type 2 and 6.3% with type 1 diabetes). Barriers include poor healthcare unprofessional marketing, lack of choice (venue, date, type of course) and low motivation to prioritise health. In 2018 Health Innovation Network (HIN) coordinated a bid for transformation funding for diabetes education across 12 Clinical Commissioning Groups (CCGs), led by Southwark CCG.

A digital first approach

Taking a digital first approach, the Priority platform was deployed to deliver an online booking system covering 12 CCGs in south London connected to EMIS Health and in time, Vision Healthcare under the Diabetes Book & Learn brand – also created by PDH.

Pulling data from the patient record at point of referral and making it available to the education course providers, patients are able to book and attend any course of their choice safe in the knowledge that their data is secure and ready for the provider when they start their course.

And course providers are able to create, edit and manage their own courses through the platform from start to finish.

Results

Since launching on 1st September 2018 the Priority platform has been powering the integrated diabetes education booking service for south London – Diabetes Book & Learn, covering a population of 3.2m.

  • Patients are able to select a course anywhere in south London through the Priority Platform
  • 41% of patients select a course outside of their CCG
  • The platform has increased outcome reporting by 75%
  • 38% of patients self-manage their bookings entirely online saving administration time
  • Over 11,000 GP referrals since launch
  • 90% of all referrals resulted in a course booking

Diabetes Education Booking Service win in south London

Major new Diabetes Education Booking Service contract awarded

The twelve south London Clinical Commissioning Groups, led by NHS Southwark CCG, along with the Health Innovation Network, the Academic Health Science Network (AHSN) for south London, are delighted to announce the award of a three-year contract to Spirit Healthcare in partnership with Priority Digital Health to help people with diabetes to select and book the most appropriate diabetes education course for them.

From October 2018, for the first-time, people with diabetes will be offered the choice of any diabetes education course available across south London. This will give them a much wider range of course location and timing options. This will provide the best possibility of increasing attendance rates and helping people with both Type 1 and Type 2 diabetes to take more control of their own health.

Across south London records show that approximately 165,000 people have diabetes, and this is growing significantly every year. With current attendance rates for diabetes education lower than 10%, the partners led by NHS Southwark CCG, sought a solution to increase uptake of new, accessible education services. During the three years of the new contract, over 40,000 people are expected to benefit from the ability to access a wider range of courses in any part of south London.

Spirit is working collaboratively with Priority Digital Health, a forward thinking digital solution provider based in Cambridge, to develop and deliver this innovative booking service.

GPs and healthcare professionals will be able to refer people with diabetes to the booking service and people with Type 2 diabetes will also be able to refer themselves and book their course online.
