Patient Management System

The PDH Platform is our proprietary 'Patient Management System' (PMS). Each module supports a range of NHS, Public Health and Community Services.

These modules include Social Prescribing ServicesLifestyle Services, Referral Management, Booking Management, Communication Management, Volunteering, Children's Services, Allied Health Services, Charity & Third Sector, Holistic Assessment, Reporting, Workplace Health and Diabetes Services.

Built by our in-house team in the UK, and designed by Co-Founder and Chief Innovation Office / CTO John Dibb, the Platform is robust, safe and flexible to meet ever-changing health, wellbeing and social care provider needs.

Tailoring

We know that every service is uniquely individual to its residents and locality. That's why every module in the PDH Platform has been designed so it can be customised for each service and every local pathway.  That way the patient / resident is placed at the heart of the service and commissioners can be assured that KPIs as well as service design requirements can be met, easily and swiftly.

Single Patient Record

One of the most important platform design principles is "Tell your story once". A single patient record where everything from the patient's contact details to their most recent holistic assessment recording is held; every step of their wellbeing journey is recorded and ensures that a patient isn't required to repeat often basic personal information again and again.

Fully Connected

The PDH Platform is fully interoperable with NHS accredited access to all the leading GP IT systems and a number of organisations across the health and social care economy in the UK. We're interoperable with EMIS, SystmOne, Vision and Microtest through the NHS Digital IM1 Pairing Integration Programme. We're also connected to NHS Digital Personal Demographic Service (PDS).

Patient Portal

At Priority Digital Health we build digital solutions that empower better self-management of people's health and wellbeing, which reduce the chances of long-term health problems. We do this by using our PDH Platform.

This is a patient management system that acts as a singular portal for all patient information, vaccinations, prescriptions, diagnoses, interactions and encounters within healthcare settings. This intelligent system can also book and refer patients to any appropriate third-party care or education service always taking a patient first approach and enabling patient choice.

Key Features

Referral Management

Referrals can be automatically sent by GP or HCP into a service facilitated by our accreditations to access the GP IT systems. Equally referrals can be received via DXS or we can create dedicated web referral forms accessible by dedicated  urls. Referrals are received directly into the PDH Platform and if required trigger a direct email or SMS to an individual inviting them to register and book with a service.

Booking Management

Appointments, diabetes education courses, weigh-ins for weight management programmes, social isolation classes - whatever your service requires our flexible booking module can help. Providers can create and update their own services including the uploading of images and information whilst patients / residents can easily find what their looking for. Our geo-location map based design also ensures that users can search easily and quickly by locality as well as by type of activity.

Communication Management

Integrated communications module with the ability to send and received SMS and email alerts and reminders. The module is designed to nudge patients / residents to attend courses, events, wellbeing reviews and for service staff to review the status of their customers.

Can support the automation of as many reminders and events as the service requires.

Patient record management

Advisors or Link Workers can set major and minor goals, record achievement of goals and set target dates with confidence levels customised for any type of service. 

Safeguarding notes and referrals are all stored with each patient / user and as that patient or user progresses on their wellbeing journey so every interaction is recorded.

Security

The PDH Platform is hosted within HSCN as standard with a leading NHS UK based provider with all servers hosted in the UK.  Dedicated logins are provided for each user using OWASP / 2FA recommended levels of security.

Reporting

We can provide access to your data in a number of ways and undertake the analytics and bespoke reports if you require them.  We have a data extraction service for larger organisations with in-house BI teams or we can integrate your data into your preferred online business intelligence platform.

Volunteering

The volunteering module is designed to connect volunteers with residents in needs of their support quickly and easily. Supporting both individual volunteers and larger special interest community organisations and charities, requests for help are uploaded onto the platform and matched, in real-time, to those volunteers who are ready and able to assist.

Holistic Assessment

Screens the user for eligibility (e.g. age, postcode), health and wellbeing status and motivation to change. Can support self assessment and available via a public service website or through an internal login.

The Platform hosts numerous accredited assessment tools including SWEMWBS, Audit and Audit C, G-PAQ, PAM and many more.

Questions are designed to move the user quickly to completion and paint a picture of their health and wellbeing from start, middle and end of their wellbeing programme.
